There's so many different rules at each level, it's hard to keep them straight. Whether it's high school, summer leagues or MLB, I have trouble keeping them straight!

I agree with Manfred on how a game is delayed. It's only a few more pitches to be thrown if a substitution is made. To bring someone in once per inning wouldn't be the best idea in my mind. Sometimes you bring a pitcher in like that, they may throw more pitches than expected. Some will do it anyway...
